The vision of One Hope Centro de Vida is to create a collective health system without obstacles, ensuring that people can obtain medical services with dignity and without barriers.

Our mission is to improve the total health of our community by partnering with our neighbors to provide affordable healthcare, follow-up, education, and spiritual guidance in a friendly and welcoming environment.

All services rendered by One Hope are staffed and managed by Community Health Workers. All services are appointment only.

Our Services and Community Initiatives Include:

  • Primary Care Medical clinics every Wednesday night and the second Saturday of the month. These clinics are staffed by volunteer doctors, residents, UNM medical and pharmacy students, and community volunteers.

  • Occupational Therapy every Wednesday afternoon, staffed by registered occupational therapists and OT students from UNM.

  • Diabetes classes on a monthly basis offer education and peer support. These classes are taught by medical and pharmacy students, overseen by a Clinical Pharmacist.

  • Nutrition classes on a monthly basis offer education, cooking demonstrations, recipes, and peer support. These classes are taught by a Certified Nutritionist.

  • Women’s Health clinics every third Tuesday of the month offering education and breast and cervical cancer screenings, partnering with NMDOH Breast and Cervical Cancer Program, overseen by Clinical Preceptors.

  • Women’s Health classes on a monthly basis offer a comprehensive, holistic focus on quality of life issues from cancer prevention to immigration.

  • Cancer Prevention classes on a monthly basis offer tools and strategies to prevent cancer.

  • Behavioral Health clinics every other Tuesday, staffed by a UNM Psychiatric Resident.

  • Dental clinics every Tuesday and Thursday afternoon. These clinics are staffed by volunteer dentists and dental assistants.

  • Through community partnerships we are able to connect our patients with low-cost specialty, diagnostic, and laboratory services.

  • Case management services are offered in partnership with Pathways to a Healthier Bernalillo County.

Salidas:

One of our unique features at One Hope is the salida, or exit interview that all patients complete with a Community Health Worker after an appointment.  The salidas are an invaluable tool for establishing trust with patients and ensuring their needs are met.  The salidas allow us to:

  • Check in with patients in their native language and make sure they understand their health, any issues they may have, and their treatment plan.

  • Identify external factors that may make it difficult for patients to complete their treatment plan, like financial hardships or lack of transportation

  • Find out about other concerns the patient may have, such as health issues, cultural differences, or problems in their personal life

  • Build relationships with patients to ensure they feel respected, welcomed, and satisfied.

Our Community Health Workers are bi-lingual Spanish/English.

Community healthcare means…

Managed by Community Health Workers:

Our Community Health Workers are the primary coordinators of care and serve as a bridge between doctors and patients. They bring in-depth knowledge of the community, experience in community outreach, and cultural and linguistic competency.

Volunteer Service Providers:

All doctors, nurses, and dentists are licensed providers, offering their time and expertise as volunteers.

Volunteer Assistants:

Students and community volunteers fill other vital roles, such as interpretation, record keeping, receptionist duties, and preparing/cleaning the examination stations and tools.

Community-Driven:

The contributions of the Community Health Workers are valued equally with those of the providers, and the needs of the patients determine the clinic's direction.

Affordable Care:

Patients pay a small fee for their visits to cultivate a sense of ownership in the clinic and their health, but costs are kept low and we work with any patients who cannot pay to accommodate their needs.

Collaboration with the Greater Albuquerque Health Community:

We build relationships with providers in the city who offer specialized services to our patients at a reduced cost, and we partner with local, county, and state agencies to develop new programs and advocate for policy change.

Salidas:

All patients complete a salida, or exit interview, after each appointment, to ensure they understand their treatment plans and diagnosis, and allow them to express any concerns, questions or fears.

Respectful and Welcoming Environment:

While our community members have often faced substandard care and cultural and language barriers, here they are respected and treated like family.

Culturally Appropriate Services:

We affirm and celebrate the cultural heritage of our patients, and we seek solutions to their health that incorporate rather than negate those traditions.

Holistic Care:

We address all aspects of patients’ health, as well as social or personal factors that may impede their health goals, such as lack of transportation to appointments or confusion about health care systems.

Our History

In the summer of 2005, East Central Ministries, Inc. (ECM) partnered with over 50 residents of the La Mesa and Trumbull neighborhoods of Albuquerque to hold a Community Health Fair. The event was a huge success with more than 250 neighborhood participants. From that pivotal event a long-term collaborative effort formed.

Over the next few years, ECM listened while community members talked of little to no medical access for themselves and their children, no access for dental care, and numerous obstacles where access was available such as language barriers, time and transportation constraints, financial barriers, as well as blatant substandard treatment. Through this ongoing conversation the idea of a “community-run” health center was born, and One Hope Centro de Vida Health Center was opened in September, 2006.

Since then, the clinic has grown and expanded, offering new services and community initiatives. We now have a fully-supplied medical center, 3 exam rooms, 2 dental stations, and a conference room where health classes and community events are held. We see from 70-100 patients a week for visits with service providers, and One Hope staff are always busy behind the scenes doing follow-up and making referrals.
